1) Please download this Excel file, and run in Excel using a Microsoft operating system. The toolbar will not display in Internet Explorer.

2) When the spreadsheet starts, you must answer YES to enable macros. Excel's security MUST be set to medium to run macros (In Excel - Tools/Macros/Security - then restart Excel)
